Westech offers post wraps in 4 and 6 inch up to 10 foot long so you can easily cover any existing wooden pillars. The base of any post can be accented with an eye appealing base trim piece.

Vinyl railing is easily attached to concrete patios using a steel mounting system.  It also slips over 4x4's used for decks.  All railings should be so nice.
